Motion Connected Privacy Policy

This privacy policy explains who we are and how we collect, use and share your personal information. If you have questions or concerns about our use of your personal information, please contact us using the contact information provided at the end.
Organizations that subscribe to our services may require us to show links to other policy statements relevant to their organization and use of our services. Follow those links to understand how those policies affect your personal information.
Who we are
MOTION CONNECTED LLC (“Motion Connected,” “we,” “us,” “our”) is a company headquartered in Green Bay, Wisconsin. We provide wellness program and communication services (“our services”) to organizations (for example employers, charities, and cooperatives) and their affiliates (for example their employees, volunteers, and members).
- Organizations subscribe to our services and afterwards their affiliates may use our services.
- We do not provide services to organizations whose affiliates are unaware of their involvement in our services.
- When an organization subscribes to our services, we do not require that their affiliates use our services.
- We are not in the business of selling, renting, or trading personal information with others for marketing purposes.
How we use personal information
We use the personal information we collect to:
- communicate with you,
- provide our services you choose to use,
- and understand how you use our services so that we may improve them.
To accomplish these, we blend information your organization provides, you provide, you authorize us to collect, we automatically collect, and we receive from third parties.
Information organizations provide
Organizations provide us with information necessary to establish and maintain a business relationship, and the information about their affiliates we need in order to provide services. Some of this information may be personal and identifiable.
To establish a business relationship, organizations need to provide contact information for those entrusted with collaborating to deliver our services (“organization admin”). This information will include:
- name
- title
- phone number
- email address
If your organization mandates verification checks for eligibility to access our services, they may furnish us with one or more of the following details regarding their affiliates:
- name
- eligibility ID
- date of birth
- department
Supplemental information for the Healthy Choice program
If you join the Healthy Choice program, your organization shares the following information about you for viewing in the EHP Healthy Choice mobile app and web portal:
- premium level
- health status
- care provider
- care participation
- readings
- communications
- marital status
Information you provide
When you choose to register with Motion Connected, you provide the following information to identify yourself and verify your eligibility to use our services:
- activation code
- first name
- last name
- login ID
- security question
- security question answer
- time zone
- password
If your organization mandates verification checks for eligibility to access our services, you may have to provide:
- eligibility ID
- date of birth
- department
If you require personal contact with Motion Connected (for example, for troubleshooting, to answer service questions, or to receive a shipment), you may have to provide one or more of the following:
- phone number
- email address
- device information
- software (such as browser, operating system, app version) information
If you choose to use Motion Connected graphs, charts, challenges or leaderboards, you may have to provide:
- timestamps
- steps
- activity time
- distance
- calories
- weight
- height
- systolic/diastolic mmHg and pulse
- HDL, LDL, and triglyceride
- glucose mg/dl and HbA1c
If you choose to participate in a Motion Connected scorecard, you may have to provide:
- timestamps
- description of activities
- wellness-related data
- supporting documentation
- images
- food journals
- survey answers
- observations and opinions
Your organization may request you provide us with additional information to satisfy their wellness or communication requirements, such as information about tobacco use or participation in an organized event. Contact your organization admin regarding additional information your organization requests.
Information you authorize us to collect
If you use an activity tracker for which we have an integration, you may authorize us to acquire the following data from your activity tracker account:
- timestamp
- steps
- activity time
- distance
This data is applied to your Motion Connected supported wellness program for use in Motion Connected graphs, charts, challenges, leaderboards, scorecards and reports.
Supplemental information on Google Fit
You can authorize Motion Connected to acquire Google Fit steps, distance, and activity time.
The Motion Connected and EHP Healthy Choice mobile apps (“our mobile apps”) available on the Google Play App Store integrate with Google Fit using Google’s Fitness API. If you enable and consent to this integration, the apps query activity data in Google Fit, aggregated and timestamped by day, and uploads the results to Motion Connected.
The Motion Connected mobile app queries:
- timestamp
- step count
- move minutes
- distance
The EHP Healthy Choice mobile app queries:
- timestamp
- step count
- move minutes
To collect distance data from Google Fit, the Motion Connected mobile app requires access to your device location. This permission is used only to collect your distance data from Google Fit and is not accessed when the app is closed or running in the background.
You can disable this integration at any time from within our mobile apps or your Google account.
Supplemental information on Fitbit
You can authorize Motion Connected to acquire steps, distance, calories, and activity time from your Fitbit account. If you enable this integration, Fitbit notifies Motion Connected when and for which days we should query.
You can disable this integration at any time from within our web portal, mobile apps, or your Fitbit account.
Information we collect automatically
When you visit our websites, we collect timestamp, page, public Internet Protocol (IP) address, operating system identifiers, device identifiers, browser type and language, and the website you visited before visiting ours.
If you install our mobile apps, we collect operating system identifiers, device identifiers, public IP address, time-stamped logs of data exchanges associated with our mobile apps, and usage statistics associated with our mobile apps.
If you install our Windows applications, we collect operating system identifiers, .NET identifiers, device identifiers, public IP address, time-stamped logs of data exchanges associated with our Windows applications, and usage statistics associated with our mobile apps.
The information we automatically collect is used to troubleshoot and understand how our services are being used.
Information we collect from third parties
We use Zoho Sites for our public website. We collect telemetry data in our public website such as referrer and how many times you visit a page to help us understand how you arrived and, and interact with, our public website. To learn more about Zoho, visit https://www.zoho.com/.

We use Google Analytics to help us understand how you are interacting with our websites, such as referrer and how many times you visit a page. To learn more about Google Analytics practices, visit https://www.google.com/analytics.
We use OneSignal to deliver and receive responses from push notifications. We also collect time stamp, operating system identifiers and device identifiers to troubleshoot and understand how our push notification services are being used. To learn more about OneSignal, visit https://onesignal.com/.
We use TypeForm as a custom forms platform for providing educational materials. To learn more about TypeForm, visit https://www.typeform.com/.
We use Facebook, Twitter, LinkedIn, and Blogger for social media. You can connect with and follow us on these sites. If you connect with us, we may collect information about you and your social media account such as names or comments. We use 3dcart to provide an ecommerce store. When you choose to use the ecommerce store, we provide 3dcart with your first name, last name, email address, and a group code that identifies your organization’s purchasing program. We use JotForm as a custom forms platform for the collection of data for interim projects that provide additional program services, such as a program satisfaction survey. To learn more about JotForm, visit https://www.jotform.com/.
We use Mailchimp for email newsletter distribution and email marketing, and to help us evaluate their effectiveness. To learn more about Mailchimp, visit https://mailchimp.com/.
Information sharing
You may choose to make certain details about yourself, such as your nickname and relative performance, visible to other users of our services. Any personal information you make visible to other users can be read, collected, or used by other individuals in ways we are unable to control or predict, such as to send you unsolicited emails. We are not responsible for any personal information you choose to share.
We limit the personal information we share with organizations to the minimum necessary to monitor their affiliates’ progress towards meeting the goals of their wellness program, educational materials, and communications.
We anonymize and aggregate information. We share anonymized and aggregated information for research, to improve our services, and to market our services.
We may share your personal information with third parties to provide you with services on our behalf. We require these third parties to protect the personal information we provide and they are not authorized by us to use or disclose your personal information except as necessary to perform services on our behalf, as specifically authorized by us or by you, or to comply with legal requirements.
Applicable law may require us to disclose your personal information if: it is reasonably necessary to comply with legal process (such as a court order, subpoena, or search warrant) or other legal requirements; disclosure would mitigate our liability in an actual or threatened lawsuit; it is necessary to protect our legal rights, our users, customers, business partners, or other interested parties; or it is necessary for the prevention or detection of crime (subject in each case to applicable law).
Cookies
Some of the cookies set while using our services are set by us and some are set by third parties who are providing services on our behalf. We use cookies in the following manner:
- Security cookies recognize you when you when log in to our websites.
- Preference cookies personalize your experience.
- Analytical and performance cookies allow us to recognize and count visits, learn how visitors use our websites, and improve our services.
Modifying and deleting your personal information
You can modify your personal information by logging into your Motion Connected account and accessing your account information.
You can have your account and any personal information deleted from our service using either of these methods:

- Visit your account information and follow the instructions under Delete Account.
Email communications
You can modify your email communication preferences for our services by logging into your Motion Connected account and accessing your account information.
Our marketing emails tell you how to opt-out of receiving further marketing emails.
You may also contact us at support@motionconnected.com to let us know that you no longer wish to receive marketing emails.
We may still send you non-service and non-marketing emails, such as emails about our business dealings with you.
Do not track
Motion Connected does not monitor or follow any Do Not Track browser requests.
Links to other websites
We may provide links to other websites that operate independently from us. We are not responsible for the content, use of, and privacy practices of these websites.
Data storage, transfer and security
We maintain reasonable safeguards designed to protect personal information against loss, misuse, or unauthorized access. We use a variety of techniques to protect personal information, such as firewalls and full disk encryption. We restrict access to your personal information to those who require it to provide our services to you.
If we are involved in a merger, acquisition, or sale of assets, we will continue to take measures to protect the confidentiality of personal information and give affected users notice before transferring any personal information to a new entity.
Personal information we collect may be retained for as long as needed to fulfill legitimate business purposes, for a period specifically required or allowed by applicable regulations or laws.
Information from minors
People under 18 (or the legal age in your jurisdiction) are not permitted to use our services on their own, and children under 13 are not permitted to register for an account or use our services.
Consistent with the requirements of applicable law, if we learn that we have received any information directly from a minor without his or her parent’s or legal guardian’s consent, we will use that information only to respond directly to that child (or his or her parent or legal guardian) to inform the minor that he or she cannot use our services. We will subsequently delete that information.
California Privacy
Motion Connected acts as a “service provider” under the California Consumer Privacy Act (“CCPA”) and our receipt and collection of any personal information is completed on behalf of the organization subscribing to our services in order for us to provide the services. Please direct any requests for access or deletion of your personal information under the CCPA to your organization’s subscription administrator.
Modifications
We reserve the right to change or add to this Privacy Policy as needed and will post any revisions on this website. We will post a prominent notice on this website to notify you of any significant changes to this Privacy Policy and will indicate at the top of the notice when it was most recently updated. We encourage you to check back regularly to review the latest version.
